Forms for Faculty
The PLAN FORM*, due October 15 of each academic year, is used to describe the learning outcome that will be assessed for that academic year.
The RESULTS FORM*, due April 1 of each academic year, is used for describe the results of assessing the learning outcome. (See sample completed forms for suggestions.)

Overview
Program-level student learning assessment emphasizes the discipline-specific knowledge, skills, and dispositions faculty expect of graduates of their programs. Beginning in the 2012-203 academic year, all academic programs will assess one of their student learning goals annually. See the full timeline for details; key dates include:
- October 15: Select one learning outcome to examine for the year and share with PACC (plan form)
- Fall semester: Collect data on the learning outcome
- Spring semester: Analyze data and, as necessary, take action to improve teaching and learning
- April 1: Share results with PACC (results form)
- May 31: Receive individual feedback from PACC
