PERSONNEL POLICIES, PROCEDURES, AND PRACTICE
Change of address and/or a change of personal status (such as marriage, separation, divorce, etc.) should be immediately reported to the Human Resources Department on a W-4 form. Change of address should also be given to the individuals immediate supervisor.
The Change of Address notification is necessary to keep personnel records current, while a Change of Status notification (and completion of W-4 form, and a health benefits enrollment card, in the Human Resources Department) is necessary became an individuals health benefits coverage is affected by such change.
