Diagnostic Assessment Evaluations

 

Project Mentor, as a regional center, provides a limited number of diagnostic evaluations for eligible college students, both within and outside of the institution. If you need updated documentation, initial evaluations and consultations, please contact our office to make an appointment for a referral for service.

Eligibility:

Student must be an undergraduate currently enrolled in collegiate study at the referring institution leading to a degree or certificate.

For colleges and universities to be eligible to use this diagnostic evaluation service, a representative from the institution must have attended a training session held by one of the five regional centers serving students with learning disabilities. Project Mentor provides training for interested New Jersey college and university disability support staff. Additional training may be required in subsequent years to maintain eligibility.

Submission of referral:
All applications must be completed by both student and referring provider. Submit all application papers along with a check or money order of $125.00, made out to New Jersey City University. Additional documentation, such as previous evaluations, may also be submitted.
