Once you have signed in to GothicNet, you will find a block link on your dashboard grid called "My Tasks". If you have tasks that need to be completed the image symbol in this block will be an orange triangle with an exclamation point. Click on this "My Tasks" block link.
After clicking on the link, you will be brought to a dashboard with task links. Under the "To Do" link is a task called "NJCU Covid-19 Vaccine Confirmation". Click on this task link to continue.
The next screen you will be presented with is "Step 1 of 3". This is the Introduction, which outlines the vaccine policy, requirements, and options. Review this page and then click the "Next" button link to move to the "Step 2" screen.
The Step 2 screen will present you with Your ID Number and name, along with a question as to whether or not you have received your first COVID-19 vaccination. Use the accompanying dropdown box to select YES or NO. Use the save button at the bottom of the page after entering your answer. Then click the "Next button" link.
You will now be asked to upload a photo of the vaccination card you were given after receiving your first dose. The information on the card should be clearly visible. Please do not upload any additional medical information or documents in this area. Once you have uploaded the image of your card, please click "next" to proceed to the Step 3 screen.
The Step 3 screen will confirm that you have submitted your information. As a final step, please click the "Submit" link on this page to complete your submission process.
Important information for anyone who has chosen "No" as their answer in Step 1:
If you have a medical exception, select NO. Access the "Link for Health Record" on this screen and complete the form. Once you have selected NO, click on "Next" to continue and complete the task.
